330 | |a Bien que les traitements immunosuppresseurs aient permis une forte diminution de la survenue de rejets aigus, la dysfonction chronique d allogreffe pulmonaire (CLAD) reste un challenge thérapeutique de taille pour la survie à long terme du greffon. La cohorte COLT créée dans le but d étudier les mécanismes physiopathologiques des Syndromes Restrictif d'Allogreffe (RAS) et de Bronchiolite Oblitérante (BOS), la forme de CLAD la plus courante, nous permet l analyse des cellules immunitaires circulantes des patients transplantés de la transplantation (TP) à la survenue du CLAD.Le profil lymphocytaire de 6 patients stables (STA) et 8 BOS a été analysé avant TP, 1 à 6 mois après TP, 6 mois avant le BOS et lors du diagnostic du BOS. Les PBMC ont été étudiés par cytométrie en flux après marquage classique des populations lymphocytaires T : CD3, CD4, CD25, CD45RA et FoxP3. CXCR3, CCR6, CD39, CD73, CD15s et PD1 ont été ajouté afin d étudier les sous-populations T helper (Th) et T régulatrices (Tregs).Cette étude ne montre aucune différence significative entre les patients qui développent un BOS dans les 3 ans post-TP et les STA dans les proportions des populations T CD4 et CD8 communes. Nous avons identifié une augmentation des Tregs FoxP3+ 1 à 6 mois post-TP chez les BOS comparés aux STA (STA - 0,85% et BOS - 3,36%, p<0,005). Cette différence a été validée sur 37 patients STA et 24 patients BOS (STA - 1,61% et BOS - 3,18%, p<0,001). L analyse des sous-populations Tregs démontre que l augmentation des Tregs chez les BOS est principalement due à une augmentation de sous-populations mémoires CD45RA- et CD39+. Enfin, contrairement au rejet aigu, la survie au BOS est augmentée significativement chez les patients présentant un faible taux de Tregs 1 à 6 mois post-TP (hiTregs - 41% et lowTregs - 75%, p<0,01).En conclusion, nous avons identifié les Tregs comme potentiel biomarqueur prédictif de la survenue de BOS après transplantation pulmonaire chez l Homme. | ||
330 | |a The irreversible chronic allograft dysfunction (CLAD) remains a therapeutic challenge for the long-term graft survival in lung transplantation (TP). For Bronchiolitis Obliterans Syndrome (BOS), the main form of CLAD, both immune mechanisms involved and predictive biomarkers have to be better defined. The COLT (Cohort in Lung TP), initiated in order to understand the pathophysiology of CLAD, gave us the opportunity to follow the immune profile of the patients from the TP to the BOS occurrence.The lymphocyte profile of 6 stable (STA) and 8 BOS patients was investigated before TP, 1 to 6 months after TP, 6 months before BOS and at the BOS diagnosis. PBMCs were analysed by flow-cytometry with the classical T-cells markers: CD3, CD4, CD25, CD45RA, and FoxP3. CXCR3, CCR6, CD39, CD73 CD15s and PD1 were also added to go deeper in the helper (Th) and regulatory T cells (Tregs) subsets. Monitoring doesn t show any significant difference between patients who will report a BOS in the 4 years and STA in the proportions of common CD4 and CD8 T cells subsets. We highlight an increase in FoxP3+ Tregs proportion 1 to 6 months post-TP in BOS compared to STA (0,85% and 3,36% in STA and BOS respectively, p<0,005). We validated this profile on 37 STA and 24 BOS patients (1,61% and 3,18% in STA and BOS respectively, p<0,001). Then, the analysis of the Tregs subsets denotes mainly memory Tregs (CD45RA- and CD39+) are responsible for the Tregs increase found in BOS patients. Finally, we show that unlike Acute Rejection, the BOS survival is significantly increased in patients with a low proportion of Tregs 1 to 6 months post-TP (41% and 75% in hiTregs and lowTregs respectively, p<0,01). In conclusion, we lead to identify Regulatory T cells as new potential predictive biomarkers of the BOS including both phenotypical and functional modalities in humans. | ||
